I declare that I am in good physical and mental condition to participate in this training and will inform the trainer if I feel unwell or unfit at any time. I agree to strictly follow all safety instructions, precautionary signs, and directions from the trainer or authorized personnel, and to avoid any unauthorized or unsafe activities during the training. I understand that failure to comply may result in suspension or removal from the program. I acknowledge that I participate at my own risk, and the Company shall not be held liable for any harm unless due to gross negligence or willful misconduct. I accept and agree to these terms voluntarily.

Saya mengisytiharkan bahawa saya berada dalam keadaan fizikal dan mental yang baik untuk menyertai latihan ini, dan akan memaklumkan kepada jurulatih sekiranya saya berasa tidak sihat atau tidak layak pada bila-bila masa. Saya bersetuju untuk mematuhi semua arahan keselamatan, tanda amaran, serta arahan daripada jurulatih atau pihak yang diberi kuasa, dan tidak akan melakukan sebarang aktiviti yang tidak dibenarkan atau berbahaya sepanjang latihan ini. Saya faham bahawa kegagalan untuk mematuhi peraturan ini boleh mengakibatkan penggantungan atau penyingkiran daripada program ini. Saya mengakui bahawa penyertaan saya adalah atas risiko sendiri, dan Syarikat tidak akan bertanggungjawab terhadap sebarang kemudaratan kecuali akibat daripada kecuaian melampau atau salah laku yang disengajakan.
